Assist-Life Stage, Transition

In life, there are so many transitions that we go through and having support during these changes can make a world of difference. As someone living with disability, it’s important that you surround yourself with a supportive network especially during life’s unpredictable and transitory stages. Assist-Life Stage Transition is a service that we offer to help you with life changes such as starting a new job, moving into a new home, starting college, changes in the family, relationships or even changing providers and supports in your NDIS plan. We provide mentoring, assistance with decision making, budgeting, upholding tenancy obligations and developing skills directly related to your new life stage

Daily Personal Activities

Your NDIS funding for Core Supports will cover Assist Personal Activities, which we highly recommend if you are unable to manage daily personal care independently. This support will allow you to plan each day and live with peace of mind, just knowing that you have someone to help you with tasks related to your personal care. It can include mobility around the house and wheelchair assistance, meal preparation, help with personal hygiene and getting dressed. Whether you need to go to work or school everyday or even if you spend most of your days at home, having support with personal care activities can ensure that you look and feel your best each day.

Community Nursing Care

For participants who have clinical needs but are looking to manage those at home, community nursing care is a great solution. With dedicated registered nurses on our team, we are able to provide this service to you in your home with an excellent level of professionalism. Community nursing care allows you to be cared for and supervised by a qualified nurse in the comfort of your home. It can also include overnight nursing or 24/7 nursing services for participants with high care needs. At LuJenn Care we take great care to offer you nursing supports that are delivered with compassion, respect and expertise. Our nurses are up to date with latest medical treatments and technology and can assist you with supervising medication administration, wound care, post-operative care, catheter feeding, bowel management, palliative support, diabetes management and more.

Daily Tasks/ Shared Living

This support is specifically geared towards participants who require in-home assistance in a house where there are roommates or housemates sharing the space. Having a disability can pose some logistical challenges when living in a shared environment, so we help to ease these barriers and facilitate a harmonious living arrangement for everyone’s comfort. Our support workers will be by your side, helping you with meal preparation, feeding, bathing and toileting, getting dressed, tidying up and fulfilling your share of household chores. We help with mobility around the house and in the garden to ensure that you can make the most of your environment. We can help build a friendly culture with housemates to make sure there is healthy communication and a fun atmosphere at home.

Innovative Community Participation

Innovative Community Participation is a support that helps you foster deeper connections within your local community. It allows a broader spectrum of participation than regular community participation supports and is focused on your overall personal growth and strengthening of self-worth. LuJenn Care offers a comprehensive service that may include connecting you with career workshops, support groups, wellness classes and other capacity-building activities that may also involve recreational activities. With Innovative Community Participation in your NDIS plan you can expect to build resilience and discover more about yourself. You can also learn more about self-care and emotional regulation and how to adapt to different situations. As a person living with disability, this kind of support is integral to your self-awareness. We help you share your particular skills and talents, whether it’s at the workplace, in your family or in a social setting.

Development-Life Skills

We assist you to develop skills to reach your full potential and live an independent life. These supports help you to go out into the world confidently and safely, using public transport, shopping for your own groceries and staying organised at work. We also help you learn life skills that can benefit you at home, such as managing your own personal care or cooking nutritious meals for yourself with minimal assistance. You can learn how to budget, plan your day and understand your own supports so you have better control over your life and can make informed decisions. We’ll also help you to work on communication and relationship skills so you can enjoy social settings with ease.

Household Tasks

Having a support team to help you manage household tasks can make a real difference in life. We have friendly, trained disability support workers who will come to your home as often as you need, consistently and on schedule. Our team will help you to clean the house, tidy the garden and help you with meals. We will assist with laundry, making the beds and general cleaning. We focus on empowering you to build your skills in these areas so that you can maintain your household as independently as possible.

Participate Community

If you have a talent or desire to learn new things or make new friends, we can connect you with clubs, programs, groups or events in your community. From swimming classes, to pottery to cooking lessons, beach tours, volunteer groups and more. Our goal is to help you to be active and engaged in your community, making friends and meeting like-minded people. This helps to build your confidence and self-esteem while also boosting your skills and developing your talents.

Specialised Supported Employment

Specialist Supported Employment is funded by the NDIS for participants with slightly higher intensity needs who may need extra help with anything related with employment. This is when your requirements exceed what is normally expected from the employer. Employers in Australia are supposed to provide a reasonable amount of support for staff who have disabilities, but when your needs are more significant, a provider such as LuJenn Care can help you achieve your employment goals.

This could involve helping you find employment in an Australian Disability Enterprise (ADE) where it’s easier for you to receive employment supports. However, you can use your Spec Support Employ funding in wider settings too, including self-employment or work at a social enterprise. Speak to us about specific specialist supports that you need in the work environment and enjoy better choice and control over your career goals.
